Registering As A Patient


The Old Machar Medical Practice is currently accepting patients within our practice boundary who do not already have access to a GP Practice.

At present the Old Machar Medical Practice (both King Street and Jesmond branch) is accepting registrations for patients that are within our practice boundary and are either new to the area or do not already have access to a GP practice.

At present we are NOT registering patients already registered with a local GP 

Please Note: This includes people who are choosing to move practices but remain living within their current practice boundary. If individuals have moved from one part of the city to another that is not covered by their current GP practice, or are newly arrived in Aberdeen, they will be registered.

Registration Requirements

Any person permanently resident within our practice boundary may apply for registration with the practice. When registering, it is essential that we see two forms of identification.

1. One form of identification should be photographic where possible or a birth certificate, and

2. the other form of identification must confirm your current address e.g. utility bill, rental agreement, or bank statement.


This is a requirement for ALL persons registering with the practice.

You must complete a registration form for each person registering with the Practice.

On completion of this registration form, you will be emailed a copy of your submitted form to the email address you provided.  Please contact the practice if you do not receive a copy of the completed form.

In order for us to complete your registration, YOU MUST reply to the email you receive with a scanned image or photograph of the following forms of identification:

Photographic Identification - for example a passport or driving license (if you are unable to provide photographic identification, a birth certificate is required)

Proof of Address - for example a utility bill, lease agreement or bank statement


The email you receive will be sent from "gram.oldmachar.registration@nhs.scot".


If you do not have an email address or you are unable to send the documents by email, you can bring the documents in to either the King Street or Jesmond surgery for us to view them in person.

Preference of Practitioner 

Since April 2004, all patients in the UK are now legally registered with the practice and not an individual GP. For administrative and continuity reasons however the Old Machar Medical Practice has decided to retain the practice of assigning each patient to a "Usual Doctor" who is ultimately responsible for their medical care.  You can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis.  We will do our best to respect your choice however, not all the doctors in the practice provide all services and specific doctors may not be immediately available.  The practice retains the right to insist that certain conditions/patients should be seen by specific practice personnel.
